WELCOME New TEAM Abilities Client!
We hope you enjoyed your tour! Enclosed you will find the new client paperwork. Please fill in all information completely. The new client enrollment packet must be completed, signed and returned in order to attend the day hab.
In order to complete your file, we will need the following:
Completed information form
Field Trip Permission Form
Current Identification (if applicable)
ALL current medication and dosage, even if medication is administered only at home
Emergency contact information
Special needs instructions if applicable
Provider company name, contact and phone number
$75 annual supply fee
If we do not have completed paperwork on the first day the client is to begin the day hab., they will not be able to attend.
It is imperative that you contact your service provider and inform them you have chosen TEAM Abilities, Inc. for your day hab. service. Once they have been in contact with us, we will send them a contract. Please allow 2-3 business days to complete this process.
